RHP 8 Plan Certification
As part of the plan resubmission process, each RHP was required to gather updated information and signatures from every organization that may provide and/or receive funding related to waiver activities within its region. Anchors, IGT Entities, Performing Providers (receiving DSRIP funds alone or both UC and DSRIP funds), and UC-only Providers all reviewed and certified their participation in the region. The list of all certifying RHP 8 organizations and their designated signatories, grouped by participation role, can be found below.
Section VI. RHP 8 Participant Certification
Each RHP participant listed below will be providing State match or receiving pool payments and has signed the RHP 8 Certification Form, certifying the following facts:
- Signatory is legally authorized to sign the document on behalf of their organization;
- Signatory has read and understands the document;
- The statements on the form regarding the signatory's organization are true, correct, and complete to the best of signatory's knowledge and belief.
