Cohort Work Group
The Cohort Group's intent is to identify new, and improve upon, existing communication pathways for providers and patients related to resources and services in RHP 17. Providers have discussed challenges in providing primary care and behavioral health and have expressed the need for better understanding of current available resources and how to access these resources. Providers also highlighted that multiple provider types, behavioral health providers, hospitals, and physician groups would benefit from increased coordination of internal and external resources, especially in addressing high-risk issues like chronic pain management in Medicaid and uninsured patient populations. To address the identified need for resource coordination, the group will first focus on identifying internal resources and increasing communication among participants, and then expand the effort to include community partners.
