Learning Collaborative

Collaborative learning occurs when a group of individuals or organizations share information and experiences in an effort to drive change and seek improvement for a common purpose. The RHP 17 Learning Collaborative seeks to achieve regional impact through shared learning, community engagement, and celebration of success. We believe there can be great benefit to the region at large when stakeholders come together in activities designed to foster collaboration and dialogue geared toward learning and continuous quality improvement.

The design of the RHP 17 learning collaborative is a version of the CMS-recommended IHI model, modified to accommodate our smaller, more rural region. The collaborative structure is very similar to that of our sister region, RHP 8 in Round Rock, and our larger regional counterpart in Houston, RHP 3, to allow participation across regions by interested providers.

"Individual" and "Regional" mechanisms for learning and sharing, fed by "Core" components, work in a continuous cycle to promote quality improvement and positive regional impact as illustrated below.

RHP 8 and 17 Virtual Learning Collaborative Event Series - May 2021

The RHP 8 and RHP 17 will once again be holding a series of virtual learning collaborative events during for DY10.

The first event, Self-Care & Burnout, was held on May 20, 2021 and was an interactive presentation hosted by Mr. Gustavo Molinar, Mental Health Literacy Specialist, with Mental Health America of Greater Houston.

Mr. Molinar provided an interactive presentation and training on recognizing burnout and implementing self-care. Participants learned about the risk of burnout, how to care for oneself, treat burnout and maintain balance.

Contact the RHP 17 Anchor Team to obtain a copy of the sign-in sheet for this meeting.

Peer-to-Peer Opportunity:

Stay tuned for any upcoming peer-to-peer learning collaborative events happening in other regions.

Regional Work Group

The Cohort work group's aim is to bring together DSRIP providers and other RHP stakeholders, who have similar projects, goals and interests in identified focus areas. The objective of this group is to foster collaboration through focused quality improvement activities in an effort to promote efficiencies and improve health care delivery and access throughout the region.

The RHP 17 Anchor Team assists in facilitating discussions amongst providers and community partners. The group works collaboratively to establish identified goals and aims.
